Patient Entry

Patients enter the referral network through informational websites focused on dental implant education and treatment options. These sites are designed to help patients understand available procedures before requesting a consultation.


Referral Request Intake

Patients complete a short referral request form that captures:

  • treatment interest (e.g., dental implants, mini implants, implant-supported dentures)
  • location (city/state/ZIP)
  • timing or urgency
  • preferred contact method (phone, text, or email)

No medical history, diagnosis, or clinical assessment is collected.


Referral Routing

Referral requests are routed based on:

  • geographic relevance
  • services offered by the practice
  • provider availability

Referrals are shared selectively and are not resold.
One patient → one practice → one conversation.


What Providers Receive

Practices receive:

  • patient contact details
  • stated treatment interest
  • timing expectations
  • preferred contact method

Practices decide how and when to follow up.


Qualification Standards

A referral is considered qualified when:

  • the request form is complete
  • valid contact information is provided
  • the patient has expressed clear interest in consultation

This referral network is intentionally limited and is designed to support patient follow-through and successful consultation experiences.

Participating practices agree to basic referral-handling standards focused on:

  • timely communication
  • clear consultation processes
  • respect for patient preferences
  • professional scheduling and follow-up

These standards exist to support both patients and providers. Clinical decisions, treatment recommendations, and financial arrangements remain solely between the patient and the dental provider.
